To do this, you need to know who to contact to get your questions answered as quickly as possible.

You can find your assigned Client Services Case Manager by matching your last name to the Caseload Assignments of each Case Manager. Our Office Hours are Monday through Friday from 8:30am to 5:00pm. If you are calling during office hours, please always ask to speak with your Assigned Case Manager or a member of your Case Manager’s Team.
After-Hours Non-Emergency Calls: If you are calling After-Hours and your call is not an emergency regarding your care services, please enter the Telephone Extension Number of your Case Manager and leave a detailed voice message and your call will be returned the next business day.
After-Hours EMERGENCY Calls: If you are calling After-Hours and your call is a non-medical emergency, please contact us at 1-877-747-5090 and at the prompt you will be directed to "Press 1" for emergency contact. You will be connected to our After-Hours Answering Service who will then connect you to the assigned After-Hours Case Manager.
Please note that if your After-Hours telephone call is regarding a Medical Emergency, please call 911 Emergency Services before contacting First Choice In-Home Care.
