Get Started with In-Home Senior CareMaking the decision to place a Home Care Aide with a loved one can be a very difficult decision. We make every effort to make this process as organized and effective as possible.

Schedule A Time To Meet With Us At Our Office

To get stated we always prefer that you meet with us in our office first as this provides you with the opportunity to meet our administrative staff and to better understand the professional manner in which we provide care services.

The In-Home Visit

The next step is to schedule an in-home visit allowing First Choice to meet with the individual in need of care and to become acquainted with the care environment. In-home visits are completed by a Home Care Program Manager. During the visit we will discuss your care needs, scheduling requirements and your needs and expectations of the Home Care Aide. Most importantly, we will answer all of your questions and be certain that First Choice is able to perform the care services you require.

Development of The Plan of Care

After we have completed our in-home visit we will begin the process of completing your personalized Plan of Care. The Plan of Care will be used to direct the care services provided by our Home Care Aide. You will be asked to approve the Plan of Care prior to care services being started. Your input to the Plan of Care is of great importance.

Placement of The Home Care Aide

At First Choice we prefer that one Home Care Aide be assigned to provide all of your care services. In the event you require more than eight-hours of services per day, two or more Home Care Aides may be assigned to provide your care services. In placing a Home Care Aide with you, we prefer to work as a team, wherein, we will find the best available Home Care Aide who has the ability and availability to provide services, but we would like for you to participate in making the final hiring decision. We start this process by scheduling an interview between you and the Home Care Aide. If the interview is successful, the Home Care Aide is scheduled to provide services. If the interview is not successful, we will continue the interviewing process until a perfect match has been made.
